Google Ads vs Facebook Ads for Chiropractic and Wellness Clinics
Short answer: Google Ads captures people already searching ("chiropractor near me", "sciatica relief"). Meta ads create demand among locals who aren't searching yet. Most clinics should start with Google search and test Meta with an offer once tracking is set up safely.
Side by side
| Google Ads (Search) | Meta Ads | |
|---|---|---|
| Intent | High: people actively searching | Lower: interrupts scrolling; good for offers, events, awareness |
| Pricing model | Pay per click; you set budget and bids | Auction; pay mostly per impression or result |
| Health targeting limits | Health is a sensitive interest category under Google's personalized-ads policy: advertiser-curated audiences (remarketing lists, Customer Match, custom segments) can't be used, but predefined Google audiences and keywords can. | Meta restricts health and wellness advertisers' use of some lower-funnel events and flags custom conversions that suggest health conditions. Expect to optimize on upper-funnel events or measure bookings outside Meta. |
| Best first campaign | Search ads on "chiropractor + [city]" keywords to a booking page | Local-radius ad with a clear new-patient offer, measured by bookings in your own system |
The HIPAA trap: tracking pixels
Neither Google nor Meta signs a BAA for ad tracking. In 2022 HHS warned that tracking technologies on healthcare sites can disclose PHI, and updated that guidance in March 2024. In June 2024 a federal court vacated part of it (the portion on unauthenticated pages), but pixels on patient portals, intake forms or booking flows that reveal health details are still a real risk. Keep pixels off intake, portal and condition-specific booking pages, and never put health details in event names or URLs.
A simple plan for a 1–3 doctor clinic
- Fix your Google Business Profile and booking page first (see local SEO guide).
- Run Google Search on a small daily budget with tight local keywords and negative keywords (jobs, school, free).
- Measure booked new-patient visits, not clicks.
- Then test one Meta offer campaign within a few miles of the clinic.
- Kill whatever doesn't produce booked visits after a fair test.

FAQ
Should a chiropractor use Google Ads or Facebook ads?
Start with Google Search to capture people already looking for care nearby, then test Meta ads with a clear offer once tracking is set up safely.
Can I retarget website visitors with health ads?
On Google, health is a sensitive category, so advertiser-curated audiences like remarketing lists can't be used for personalized health ads. Meta also limits health advertisers' data use. Plan for broad local targeting.
Is the Meta pixel HIPAA compliant?
Meta doesn't sign a BAA for its pixel. Keep pixels off pages that collect or reveal health information, such as intake forms, portals and condition-specific booking flows.
How much should a clinic spend on ads?
There's no universal number. Start small, measure cost per booked new patient, and scale only what works.
