Best Chiropractic Social Media Marketing Agencies (2026): Honest Ratings
Checked October 15, 2026, from public sources only (agency websites, published terms and public review sites). We did not contact any agency, and nobody paid for placement. Scores are provisional: they use published information only, not hands-on client experience.
Chiropractic marketing is a low-trust space, so we focus on what agencies actually publish. We researched 19 agencies (plus one self-serve ad tool) and scored 11 of them with our published agency rubric: specialization 25%, pricing transparency 20%, verified results 25%, contract fairness and ownership 15%, independent reviews 15%.
The short version
- Most transparent: CHIROBASIX (86/100). Published prices, no setup fee, month-to-month marketing, written ownership.
- Most honest about results: ChiroCandy labels its numbers as ad-platform leads, not new patients.
- Fairest terms on paper: Go Chiro Pro (month-to-month, 30-day refund of management fees), though we found little independent proof.
- Lower scores mostly reflect unpublished pricing and terms. Use the red flags and questions below with any agency.

What we found in our research
Patterns from published agency terms and public practice-owner reports, anonymized. Individual reports are experiences we could not independently verify.
- One agency's contract auto-renews and charges a $999 early-termination fee plus the remaining balance.
- One agency's terms make all fees non-refundable, require you to pay the rest of the term if you cancel early, and only accept cancellations submitted by a set day of the month.
- One agency's terms let it use and sell de-identified data derived from your patient information.
- One agency's terms keep ownership of the practice videos it produces until a 12-month term is complete.
- One agency's "free" website turned out to be leased and was set to come down at the end of the contract, according to a client.
- One agency's support documentation says you may not move your website to another host if you cancel.
- Several agencies' guarantees count leads (form fills), not booked or showed patients.
- At least one guarantee leaves the qualifying conditions to be "agreed before launch", so the promise can't be checked in advance.
- Practice owners described quotes that dropped sharply during a single sales call, from several thousand dollars a month to a few hundred.
- Practice owners described agencies that kept billing after a cancellation request, or that required an extra "off-boarding" call before a cancellation counted.
- One agency advertises pay-per-case pricing, while a practice owner reported paying a large upfront setup fee.
- Some agencies show thousands of 5-star reviews at the same time that practice owners describe serious problems. Review volume alone proves little.
- Most agencies don't publish who does the work, whether ad accounts are in your name, or their cancellation terms.
Red flags we found
Long contracts and auto-renewal
12-month minimums, auto-renewal, early-termination fees, and "pay the rest of the term if you cancel" clauses.
They own your accounts
If the agency owns the ad account, the Facebook page, the Google Business Profile or the domain, you lose your history and audiences when you leave. Insist on your own Meta Business Manager and Google Ads account, with the agency added as a partner.
Rented websites and landing pages
"Free" websites that are actually leased, or sites you are not allowed to move to another host. Rented funnels mean you start from zero when you leave.
Guarantees with fine print
"100 new patients or you don't pay", where qualification is "agreed before launch". "30+ qualified leads or your money back", where leads are not patients. Trial periods with conditions that aren't in the contract.
"New patients per month" promises
No agency controls your phones, scripts, pricing or show rate. Ask whether a figure counts leads, booked appointments, showed patients or paid patients.
Shifting prices on the sales call
Quotes that drop sharply during a single call. Pricing that moves that much tells you the first number was never real.
Hidden ad-spend handling
Ask whether ad spend is billed to your own card on the platform, or passes through the agency (where it can be marked up).
Reviews that are too perfect
Thousands of 5-star reviews with no detail, or reviews that don't match what local doctors tell you, deserve a closer look.
10 questions to ask before you sign
- Will my Meta ad account, Facebook page, Instagram, Google Ads, Google Business Profile and domain be in my name, with you added as a partner?
- Is ad spend charged directly to my card by Meta/Google, or does it pass through you?
- What is the minimum term, does it auto-renew, and exactly how do I cancel (notice period, method, fees)?
- Is there a setup fee, and what do I get for it?
- If you build a website or landing pages, do I own them and can I move them when I leave?
- What does your guarantee count (leads, booked, showed or paid patients), and what disqualifies me?
- Who actually does the work: in-house staff, contractors or an offshore white-label team?
- Can I speak with three current chiropractic clients in a market like mine, named and with contact details?
- What will I see each month: cost per lead, cost per booked visit, show rate, cost per new patient?
- If you touch patient data (forms, CRM, texting), will you sign a BAA, and are testimonials HIPAA-safe?

- ☐ Ad spend: billed directly by the platform to the practice's card
- ☐ Exit: within 7 days, admin access is returned and agency access removed
- ☐ Fees: setup fee itemized; all usage charges (SMS, AI minutes) priced
- ☐ Guarantee: the metric, the timeframe and every condition written out in full
- ☐ Reporting: monthly, including cost per booked and per showed new patient
- ☐ HIPAA: BAA signed if they handle PHI; no PHI in ad targeting
- ☐ Data: no assignment of your patient data, de-identified or not, to the agency

What we could not verify
Most agencies don't publish who does the work (in-house or white-label), their ownership terms, or client counts we can check. Google review counts for most agencies could not be read reliably. Where a field says "Unknown", ask the agency and get the answer in writing.
How we researched
Web searches, agency pricing and terms pages, and public review sites. No agency paid to be included, and we have no affiliate relationship with any agency on this page.
FAQ
How much does a chiropractic social media marketing agency cost?
Published prices we verified in October 2026 range from about $298/mo (Online Chiro: $149 management plus a $149 minimum ad spend, with a $199 setup fee) to $2,290–$2,300/mo for full programs (ChiroCandy, CHIROBASIX). Meta ad spend is extra, and agencies typically recommend $1,000–$1,500 per month.
Should I sign a 12-month contract with a marketing agency?
We recommend against it. Several agencies we reviewed (CHIROBASIX, Go Chiro Pro, DC Practice Growth) offer month-to-month marketing, so a long lock-in is not standard. Websites often carry a 12-month term; make sure you own the site at the end.
Are new-patient guarantees real?
Read what is counted. Some guarantees count leads (form fills), not patients, and some leave the qualifying terms to be agreed later. Get every condition in writing.
Who should own my Facebook ad account?
You should. Create your own Meta Business Manager and ad account and add the agency as a partner, so your data, pixel history and audiences stay with you.
Which agency is best for chiropractors?
On published information alone, CHIROBASIX and ChiroCandy scored highest for transparency. Go Chiro Pro has the fairest terms on paper but little independent proof. Always call references in a market like yours.
