Best AI Receptionists for Chiropractors (2026)
Every missed call at a chiropractic front desk can be a lost new patient. This guide compares the AI and virtual receptionist options we're tracking, based only on what each vendor publishes on its own website (checked October 14, 2026). We haven't run hands-on tests yet, so nothing below is a test result. Anything we couldn't verify says unknown.
At a glance
| Tool | Type (per vendor) | Free/entry tier | Pricing | HIPAA / BAA |

| Podium | 24/7 "AI Employee" for lead response and booking | unknown | unknown | unknown |
Smith.ai
Smith.ai calls itself "24/7 AI Receptionists & Live Human Agents." Its homepage advertises a free tier of 25 calls a month, but the current homepage messaging is aimed at law firms, home services, and IT, not at chiropractic. Worth considering if: you want AI with a human backup. Unknown: chiropractic EHR integrations, whether it signs a BAA, and paid-plan pricing.
Ruby
Ruby offers live virtual receptionists and managed live chat, and lists healthcare among the industries it serves ("doctors, dentists, solo practitioners, chiropractors, and other providers"). It also mentions appointment scheduling and integrations. Ruby is human-staffed, not purely AI. Unknown: pricing amounts and BAA availability.
Arini
Arini describes itself as "the leading AI receptionist for dentists." It lists inbound call answering, patient reactivation, online scheduling, and reminders, and publishes a dental case study (Wolfe Dental) along with an ROI calculator. It isn't built for chiropractic, so how well it fits a chiro workflow is unknown. Unknown: pricing, chiropractic EHR integrations, and BAA availability.
Podium
Podium markets a 24/7 "AI Employee" that responds to leads, qualifies them, books and confirms appointments, and re-engages past customers. It says more than 60,000 businesses use it. Its current homepage is heavily focused on car dealerships. Unknown: healthcare-specific features, pricing, and BAA availability.
What to ask any vendor before you buy
- Will you sign a Business Associate Agreement (BAA)?
- Does it integrate with my EHR or scheduler (ChiroTouch, Jane, ChiroFusion, etc.)?
- Does it text back missed calls, and how fast?
- What happens with calls it can't handle, and is there a human handoff?
- What's the full monthly cost, per-minute or per-call overage, and contract length?
Our verdict (so far)
We aren't naming a winner until we've scored each tool against our rubric. From the vendors' own sites, Ruby is the only one that explicitly mentions chiropractors, Smith.ai is the only one with a published free tier, and Arini is the only one built for healthcare offices. We haven't yet confirmed BAA or HIPAA status for any of the four, so ask each vendor directly.

FAQ
Do AI receptionists work for chiropractic offices?
They can answer calls and book or capture leads around the clock. We haven't tested any of them yet, and only Ruby's site mentions chiropractors by name, so ask each vendor for chiropractic references.
Is an AI receptionist HIPAA compliant?
Only if the vendor signs a Business Associate Agreement and you configure it correctly. We haven't confirmed BAA status for Smith.ai, Ruby or Arini. Podium's terms incorporate a BAA for covered entities.
Which AI receptionist has a free plan?
Of the four we reviewed, only Smith.ai advertises a free tier (25 calls a month, per its homepage).
